Parental Consent

I. PARENTAL CONSENT TO PARTICIPATE: I, as parent or legal guardian, acknowledge that my child is in good health. I give my approval for my child to participate in basketball related activities with Skills Over Thrills Basketball Training, and Events, LLC (hereby referred to as “SOT”). For the purpose of this consent “activities” are defined as training sessions.

II. RELEASE FROM LIABILITY: I agree to assume all risks and hazards incidental to participation during any “activities” with SOT. I do hereby waive, release, absolve, indemnify, and agree to hold harmless SOT, the officers, coaches, volunteers, participants, and person(s) training my child from any and all activities for any claim arising out of an injury to my child whether the result of negligence or for any other cause or reason.

III. MEDICAL RELEASE: Because my child is involved in an active sport, there may be an occasion when an injury occurs that requires immediate medical treatment and you are unable to contact me. I hereby grant permission to SOT and/or its volunteers to secure proper treatment by contacting the appropriate medical staff and/or hospitalize my child in case of emergency, provided they are unable to communicate with me first, and according to their best judgment.

SOT will not be expected to dispense medicine to any participants with a medical condition. Only the parent(s) or legal guardian(s) of a participant may perform these duties, unless the child is capable of performing these duties themselves. If so, parents must provide SOT a letter giving the child permission to administer medication, along with written instructions on how, when, and under what conditions the child is to administer medication.

IV. EQUIPMENT RESPONSIBILITY: I, as parent or legal guardian of said participant, do hereby assume full and complete responsibility for the proper care and maintenance of all equipment provided and utilized by SOT. I agree to reimburse replacement cost of any destroyed equipment, with the payment due upon request by SOT.

V. COMMUNICATION AND PROMOTIONAL CONSENT: As a condition to my child’s participation, I hereby consent to receive communications via phone, text, and email from SOT. I understand that SOT does not sell its contact lists and communications. Communications sent may contain program information as well as special offers and may be opted out of via written request to SOT. Furthermore, I hereby grant to SOT the absolute right and permission to make, reproduce, broadcast, or otherwise use participant’s name and likeness, any photograph, videos, recordings, or other depictions or images in whatever form or media in connection with participation in SOT throughout the universe in perpetuity and in any and all advertising and promotion materials, in any manner or media whatsoever for purposes of art, advertising, editorial, trade, or promotion. To the extent that any benefit accrues or may accrue to SOT, I hereby and forever waive any interest in or claim to such benefits and acknowledge that SOT is under no obligation to exercise any rights granted herein.
